Scholarship Applications

Scholarship forms collect the applicant and contact details shown on the form and the supporting documents requested by the selected scholarship. Applications are stored in the website database, and uploaded documents are stored on the website’s hosting system. The application system sends notification emails to its configured recipients. Website accounts with the relevant administrative permissions can access submissions and download documents.

Please submit only documents required for the selected scholarship. Do not include medical records, government identification numbers or unrelated personal information. For a child under 13, a parent or legal guardian should contact FHASNE before submitting personal information.

Volunteer Forms

The volunteer form collects your name, email address, telephone number, whether you are over 18, skills or qualifications, and any optional practical participation support you request. The completed form is emailed to FHASNE to coordinate participation, and a confirmation email is sent after successful delivery. Do not include diagnoses, medical history or medical records in the optional support field. You may contact FHASNE instead to discuss arrangements. For a volunteer under 18, a parent or legal guardian should contact FHASNE about participation and submission arrangements.

Copies, Retention and Privacy Requests

Submitted information may also be present in notification mailboxes, delivery or website email logs, downloaded copies and backups. Deleting a website submission does not by itself establish that all such copies have been removed. This notice does not specify a fixed retention period for these records. Contact FHASNE for the applicable program’s retention and deletion arrangements, or to request access, correction or deletion, before submitting if you need clarification.

A parent or legal guardian should contact FHASNE before submitting personal information for a child under 13, so that the program’s submission and privacy process can be explained. Please do not include a child’s medical records or other sensitive documents in a general inquiry. Parents and guardians can contact FHASNE about information submitted for their child, including requests to review, correct or delete it.
